Dear Parents and Student Athletes:
I hope you are having a great summer! This letter contains important information on what you need to complete in order to participate in athletics at Oak Hall School.
Ticket-to-Play information, our Parent-Student Athletic Handbook (please note this contains winter and spring starting dates as well as current coaches contact information), the Parent-Coach Communication Guide, our annual parking letter with detailed instructions about available parking for athletic contests, and a flyer detailing easy instructions on how to access athletic schedules, directions, and other information pertinent to our athletic program is attached to this email.
The “Ticket to Play” form is a required form for student-athletes that is used to insure our student-athletes are eligible to play and have been medically cleared to participate in athletics. It will require the student athlete to accrue signatures on the form showing that they have correctly completed and returned the following documentation:
FHSAA Physical (EL02)-This form is attached or may be obtained by visiting our web site at www.oakhall.org , select Athletics, Required Athletic Eligibility Forms, and finally download and print the FHSAA Physical Form. Physicals are only valid for one calendar year from dated signature of practitioner who administered physical.
FHSAA Parental Consent Form (EL03) - This form is attached or may be obtained by visiting our web site at www.oakhall.org, select Athletics, Required Athletic Eligibility Forms, and finally download and print the FHSAA Consent Form. The Consent Form is only valid for one calendar year from dated signature of parent and student-athlete.
Birth Certificate-while this form may be on file in the school office, it is now necessary that you provide one for the Athletic Office. A list of birth certificates on file can be found on our web site at www.oakhall.org, select Athletics, Required Athletic Eligibility Forms, and click on the link for Birth Certificates on file. If you are a new student to Oak Hall or a rising 6th grader, you will need to submit a birth certificate, we will not have one on file nor do we have access to the student cumulative folder.
That your enrollment contract for the 2016-2017 school year has been turned into the admissions office.
The Booster fee of $200 per student-athlete has been authorized for billing. Please make sure a parent signs the Ticket to Play. This fee will be billed to your FACTS account.
The student-athlete must be eligible by OHS and FHSAA standards. Eligibility policies may be found in the Student-Parent Athletic Handbook found on the athletic link of our web page at www.oakhall.org.
The OHS Sportsmanship Contract is attached or may be found by visiting our website at www.oakhall.org, select Athletics, Required Athletic Eligibility Forms, and finally download and print the OHS Sportsmanship Contract.
Authorization to Use or Disclose Protected Health Information is being required by the University of Florida who provides for us our Athletic Training Staff and affiliated services. The form is attached or may be found by visiting our website at www.oakhall.org, select Athletics, Required Athletic Eligibility Forms, and finally download, print, and sign the Authorization to Use or Disclose Protected Health Information.
NEW requirement form the FHSAA is the viewing of the NFHS Concussion Course. All athletes must visit http://nfhslearn.com/?courseID=38000 and select the Concussion in Sports Course. Please view it, take the quick quiz, and bring a copy of the Certificate of Completion with you when you submit your Ticket to Play. This must be done annually.
Section II of the form again is a collection of information from you, however if you respond YES to any of the questions, an appointment will need to be made with me to secure your eligibility. If you need assistance with eligibility, please note Coach Malloy will not be available August 1st through August 9th.
The Athletic Office will be open and staffed throughout the summer leading up to the first day of practice. Please see the Athletic Office hours at the bottom of this email. Ticket-to-Play forms may only be signed by Coach Malloy, Coach DiBernardo, Danielle Tripp (August 1st-5th ONLY), or Buffy McTureous. It is strongly suggested that you do not wait until the first day of practice to obtain signatures. Once you have all of the above forms completed, bring the completed forms and your “Ticket to Play”, to the Athletic Office for the required signatures. Do NOT simply drop off the forms. Please do not submit forms individually. In an effort to minimize lost forms, we ask that you only come for signatures once all of your paperwork is complete. Incomplete or partial Ticket to Play forms will not be accepted or processed. You must submit them for signature and get the Ticket to Play form back as receipt for submission to your coach. Only at that time, will you be permitted to practice, tryout, attend a conditioning session, or an open facility session. No exceptions will be made to this policy in order for the Student-Athlete to be in compliance with the rules of the FHSAA and not jeopardize his/her eligibility.
Alternatively, all forms may be scanned and e-mailed to email@example.com. Forms will be checked in and a signed “Ticket to Play” will be e-mailed back to you. Please allow 2 business days to receive a signed Ticket to Play back. Please do not expect immediate eligibility upon submission. Finally, please note that Mrs. McTureous will be out of the office from August 1st-August 5th. If you submit Ticket-to-Play paperwork by email during this time period, it will take longer than two business days to receive a response from Mrs. McTureous.

FALL SPORTS FIRST PRACTICE DATES, TIMES, AND LOCATIONS

Boys’ Golf (Varsity and JV/MS boys): Practice will begin at 3:00pm on August 15th at the Gainesville Golf and Country Club clubhouse. Please come prepared to play. If you have any questions, please contact Frank Anderson at email@example.com
Girls’ Golf: Practice will begin at 3:00pm on August 15th at the Gainesville Golf and Country Club clubhouse. Please come prepared to play. In order to have a team we must have 5 committed players so please be sure to attend practice or notify coach of your inteions.
MS Football: Practice will begin August 8th at 9:00 AM to 12:00 PM at the East Field (behind the lower school). MS Players are invited to attend Varsity practice beginning August 1st from 9:00 AM-12:00 PM but attendance is NOT required.
